Patient Centered Medical Home

 Both locations of Pembroke Family Medicine are an accredited Patient Centered Medical Home.  Our staff has worked very hard to become a Patient Centered Medical Home and have received the highest level of accreditation from the National Committee for Quality Assurance.

 What is Patient Centered Medical Home?

It means our health professionals work together to provide you with a comprehensive approach to health care.  We make every effort to provide our patients with personalized attention focused on quality, satisfaction, and ways to lower costs.

Team Concept

 Our providers work in teams.  This means that we will make every attempt to schedule you with the provider you are accustomed to seeing.  If your regular provider is not available we will try schedule you with a provider on the same team. 

If you would like to see a specific provider, just ask and we will try to accommodate your requests.

Care Coordination

We are working with several community organizations to incorporate Care Coordination into our practice.  This means we will be able to provide help to our patients with chronic conditions that may need extra community services or someone to check in on them in-between visits.
